We’ll take you through the Medicare guidelines and develop a personal plan for your treatment. Our GPs can also organise care plans on your behalf.

    Care Plans

In general practice, a care plan is a process for setting and achieving health goals. This involves:

    • Assessment of the patient’s condition(s) in relation to overall health and functionality
    • Outlining the practitioner’s goals for the patient
    • Addressing the patient/carer’s needs and goals
    • Planning treatments and actions that will meet the goals, and planning review steps

If the GP involves other health and care professionals as well, the plan may also include:

    • Collaboration between team members to prepare the plan
    • Recording of each party’s goals and the treatment
    • Care planned to meet those goals, and regular review of the team arrangements

    Mental Health Plans

If you have a mental health issue, our GPs can work with Mental Health Plans offered by Medicare to plan and co-ordinate the appropriate health care for you.

Mental Health Plans are available to patients with an assessed mental disorder who would benefit from a structured approach to the management of their treatment needs.

While all our doctors deal with patients who have psychological issues, we also have two in-house psychologists:

    Sherisse Cohen
